ACHRU launches new video about post-stroke intervention

The Aging, Community and Health Research Unit (ACHRU) has released a new video, “Post-stroke intervention helps older adults transition home from hospital.”

In the video, ACHRU researchers, a stroke patient and members of the healthcare team from the Regional Rehabilitation Centre at Hamilton Health Sciences discuss how a newly developed post-stroke intervention helps improve the transition from hospital to home for older adults with stroke and multiple chronic conditions. The intervention is supported by a mobile app, My Stroke Team (MyST), that was developed by ACHRU.

The video features School of Nursing professors Maureen Markle-Reid and Ruta Valaitis.
