Submit this request through your Mosaic account if you have missed or will miss an evaluation that is worth 24% or less of your final grade in a course.
Relief for Missed Academic Work
Submit this request if you have been or will be absent for more than three consecutive days, the evaluation/academic work you will be missing is worth 25% or more (not a final exam), and/or you have already used the MSAF this term. This request should also be submitted if you will be missing academic work worth any percentage due to your involvement in a varsity athletics event/competition.
Request for Deferred Final Exam
Submit this request if you have missed or will be missing an exam during the final exam period for extenuating medical or personal reasons.
McMaster Student Health Certificate
Use this form at medical appointments in order to obtain all required information needed to validate absences.
Submit this request if you will be missing academic work during the term due to religious, indigenous, or spiritual observance(s). If you will be missing an exam during the final exam period for religious, indigenous, or spiritual observance(s) please contact the Registrar's Office.
Professional Practice Placement (Clinical) Absence
Students cannot submit an MSAF or Request for Relief from Missed Academic Work to report absences in the following courses: NURSING 1K02 A/B, 2L03, 2K02 A/B, 2P03, 2J04, 3QQ3, 3ZA3, 3ZB3, 3X03, 3Y04, 4J07, 4K10. .
When you have been or will be absent from a class/shift for any of the above courses, you must immediately notify your tutor for the course using contact information found in the course outline. In addition, notify your Placement Agency according to the procedure the Agency has outlined.
Consult Instructor of Course(s) regarding formal Accommodations
If you have formal accommodations through Student Accessibility Services (McMaster), Accessible Learning Services (Mohawk), or Accessibility Services (Conestoga) it is important that you communicate this as soon as possible to your instructor(s) in order to determine how you will be accommodated throughout the course.
